As a courtesy to our patients, we submit all claims to all health insurance companies. To ensure that your claim is submitted in a timely fashion, please make certain you bring your health insurance identification card(s) to all appointments. If your insurance is in your spouse’s name, or another person, we will need their date of birth and social security number.

All co-payments and deductibles are always the patient’s responsibility. All co-pays must be paid at the time of service.

We will file all workers compensation claims. We will require your claim number, date of injury and all workers compensation claim filing information. This information is normally provided to our team when your employer makes the initial appointment.

Payment at time of service is required! However, in certain circumstances we are always willing to discuss self pay options with you at any time. We also offer Care Credit.

We do not get involved with billing for motor vehicle or third party claims. However, we will always do everything we can to assist you, our patient, in getting reimbursed for any services performed.

Often, insurance companies require prior authorization for any diagnostic tests and/or surgical procedures. It is, therefore, important for our patient’s to understand that they are responsible for any services not covered by their insurance. We are more than happy to assist with any prior authorization requirements.
